New Family Enrollment

Welcome to the Sauk Prairie School District!

We're happy to have you join us.  New or prospective families are welcome to contact individual schools for building tours and school information. However, all enrollment occurs through the District Office. Please reach out at any time if we can help answer questions or assist during the enrollment process. 

Registration for Current School Year

Please use the links below for enrolling your student for the current year. Please submit all enrollment forms to the District Office at 440 13th Street, Prairie du Sac.

The District Office is open Monday through Friday 7:30 A.M. to 4:00 P.M. mid-August through mid-June. A drop box located to the right of the main entrance is available 24 hours a day.

It is not necessary to make an appointment to register. However, if you or your family would benefit from the assistance of an interpreter, please contact us in advance so we can make arrangements for an interpreter to be available. You may reach the District Office by calling 608-643-5990.

Enroll for 2024-25 School Year

Please use the links below for if you are planning to enroll a new student for the next school year. Please submit all enrollment forms to the District Office at 440 13th Street, Prairie du Sac.

This modified enrollment form provides only the basic information to begin the enrollment process. It is still necessary to provide supporting documentation to verify your child's legal name and date of birth, proof of residency and immunizations before the pre-enrollment is complete and the entry in Skyward can occur.   Information specific to the 2024-25 school year, such as adding/updating emergency contacts, health information, arrival and dismissal transportation plans, photo and field trip permissions, etc., will be collected during the online Annual School Registration process July 15-31, 2024.

What is Needed to Enroll/Register

Completed enrollment form (links above)

Proof of Residency can be established by presenting two (2) of the following:

  • A signed current lease which includes the landlord's name, address and phone number.

  • Any of the following documentation mailed to the resident address and dated within forty-five (45) days prior to the registration date is accepted as proof of residency

    • Auto or health insurance bill

    • Bank statement

    • Credit card statement

    • Driver's license renewal notice

    • Food Share/ Quest benefit statement

    • License plate renewal notice

    • Medicare/ Medicaid/ Badgercare benefit statement

    • Paycheck stub

    • Utility bill; gas, electric, water/sewer, cable or telephone (no cell)

Verification of Student's Legal Name and Age (please present one (1) of the following):

  • Birth Certificate

  • Passport

Immunization Record if child's immunizations occurred outside of Wisconsin
